Metformin is one of the oldest, most widely used, and least-expensive antidiabetic drugs for treatment of type 2 diabetes (T2DM). It is one of several orally-available medicines for this disease, but is very different structurally from the other compounds.

The chemical name of metformin is N,N-dimethylimidodicarbonimidic diamide, or, equivalently, N,N-dimethylbiguanide. It is usually provided as the hydrochloride salt. Metformin is marketed under several names, including Fortamet, Glucophage, Glucophage XR, Glumetza, and Riomet. When formulated with other antidiabetic agents, it is also available as Actoplus Met, (metformin and pioglitazone), Avandamet (metformin and rosiglitazone), Glucovance (metformin and glyburide), Janumet (metformin and sitagliptin), and Metaglip (metformin and glipizide).

Metformin is used to help control blood glucose levels in women with PCOS by reducing glucose production in the liver and increasing sensitivity to existing insulin. It is not a replacement for injected insulin and is not used in treating type 1 diabetes. It is often used in combination with other medications as part of regimen that includes recommendations for weight loss and decreased caloric intake.

Commonly-reported side effects of metformin include: diarrhea, bloating, stomach pain, gas, constipation.

Serotonin is to blame! Excessive release of 5HT(serotonin) from intestinal mucosa cells can cause diarrhea and nausea. Metformin stimulates these intestinal cells to produce increased levels of 5HT, as soon as 10-15 minutes after taking metformin. The good news is that most of these symptoms can improve with dietary support and a tailored nutritional and herbal strategy.

Research results indicate that metformin induces 5-HT3 receptor-independent release of 5-HT from human duodenal mucosa via neuronal and non-neuronal mechanisms. Part of the gastrointestinal side effects observed during treatment with metformin could, thus, be produced by the release of 5-HT and other neurotransmitter substances within the duodenal mucosa.
